A healthy reporting culture is an essential part of a safe and productive resources industry.

Creating a positive safety culture in the resources industry requires a positive reporting culture. Open, honest and transparent incident reporting is a cornerstone of a safe and healthy industry.

Report for Safety aims to understand the reasons why people do or do not report high potential incidents (HPIs), near misses and early warning signs and to understand the resources industry's safety reporting practices.

Over the course of the campaign, the Commissioner for Resources Safety and Health will be engaging with the resources industry to identify the opportunities and barriers related to the reporting of safety concerns.

High reliability organisations

One of the key recommendations of Dr Sean Brady’s Review of all fatal accidents in Queensland mines and quarries from 2000 to 2019 was that the industry should move towards adopting the principles of high reliability organisations (HROs).

A HRO is an organisation that has succeeded in avoiding catastrophic events despite a high level of risk and complexity in its operations. An essential characteristic of a HRO is a healthy reporting culture.
